| Approval of the Candidacy application admits the
student as a candidate for the degree. This form contains the coursework
that will be used to fulfill the course requirements for the respective
degree as agreed to by the department/program and the student. For
the masters degree, the signatures also indicate the committee
members willingness to serve; for the doctorate, the Doctoral
Committee Appointment form serves this purpose. For all but doctoral
students, the Admission to Candidacy Application is the only form
that must be submitted prior to the graduation semester. Doctoral
students must also submit the Doctoral Committee Agreement form
prior to the semester of graduation.
All students are urged to complete and submit this form as early
as possible.
Once filed, the form becomes a permanent record.
